What Happens When Your AI Tool Makes a Mistake?

Your AI tool will make a mistake. Not might. Will.

It will invent a fact, misread a document, or confidently produce the wrong answer at the worst possible moment. This isn't a flaw in your vendor selection. It's the nature of the technology. The question was never whether it would happen. The question is what your team does in the first hour after it does.

Most organizations have no answer. The mistake gets discovered by a customer, forwarded to a manager, discussed in a panicked meeting, and handled differently every time. That's how small errors become big ones.

The fix is boring and cheap: decide in advance. Who gets told? Who decides whether to pull the tool offline? Who talks to the affected customer? Who writes down what happened? Put names next to each job, not departments. Departments don't answer phones at 4 p.m. on a Friday.

Don't automate chaos. A fast tool with no failure plan just helps you make mistakes at scale. Slow down, write the plan, and then let the tool run.
