A Human in the Loop Is Not a Decoration
"Don't worry, a human reviews everything." I've heard this sentence in rooms where the human reviews two hundred AI decisions a day, clicking approve every eleven seconds. That is not oversight. That is a rubber stamp with a pulse.
Real human-in-the-loop means the person can actually say no — and does, sometimes. It means they have the time, the information, and the authority to overrule the machine. It means the organization tracks how often humans disagree with the AI, because a zero percent override rate doesn't mean the AI is perfect. It means nobody's really looking.
Don't automate chaos. If your process was messy before AI, the AI just makes the mess faster. Put a real human at the choke points that matter, and give them genuine power to stop the line.
